Steady Health is a new, specialized diabetes clinic in the Financial District of San Francisco. We are entirely reinventing diabetes care by making it a more personalized and convenient experience. Who do we serve?
We provide care for anyone who wants to gain a better understanding for their blood glucose using a Continuous Glucose Monitor (CGM), regardless of whether their diagnosis is Diabetes Type 1 or 2, pre-diabetes or gestational diabetes. Most of our care is based on remote monitoring and digital communication, and we can serve patients from anywhere in California as long as they are willing to come in for one office visit per year.
What happens after your referral?
When you refer your patient to us, we will reach out to schedule their first appointment over video or face-to-face. If they don’t have one already, we help them obtain a continuous glucose meter (CGM) and show them how to communicate and share data with us through our app for iOS and Android.
Once they are set up with these tools, we will ask them to use the app to track their diet, exercise, and medication for two weeks before coming into our office for a face-to-face consultation. We will ask them about their health goals, go over their day-to-day blood glucose management data, and highlight patterns that we have seen. At the end of the visit, we set goals together and lay out a care plan.
After the consultation, we continue to reach out regularly to encourage and maintain progress towards their goals and are available to troubleshoot whenever problems or questions arise. In the event of more complex issues, we can arrange for video consultations or face-to-face visits as appropriate.
